Beim Gameplay sticht vor allem das neue, vertikale Leveldesign hervor: Spieler werden in The Old Blood eine Menge klettern. Das Werkzeug dafür ist ein Rohr, das in zwei Teile getrennt und als Kletterhaken verwendet werden kann.
